Request for New Mail Cards
All outgoing mail requires a mail card for any postage charge. This pink card provides the required barcode to process any mailing. The card also includes the department name, account name, MoCode and people soft account number for postage.
To order new or replacement mail cards, please submit your request using this form.
Document Disposal/Shred
Departments having confidential or sensitive documents that are not records, or that are beyond their retention period, should send them to the Records Center for disposal.
Confidential and sensitive documents sent to the Records Center for destruction should be placed in boxes no larger than a standard copy paper box. If you need to request boxes, please complete a UM40 Customer Registration and Box Order Form. It is recommended that you obtain boxes as needed rather than maintaining a large quantity.
Each box sent to the Records Center for destruction must have a GREEN Records Destruction Label (UM 176A) attached to the top of the box.
For your convenience there are two versions of the file to choose from.
Print this form on WHITE paper - Shred Label - White Paper
Print this form on GREEN paper - Shred Label - Green Paper
All paperclips, binder clips, ringed binders, prongs, rubber bands, metal tabs, coil and comb bindings, compact discs, etc., must be removed prior to pick-up. Standard staples may be left in place.
Tape the top, bottom, and all seams of the box closed before requesting a pick-up.
Place shred boxes in the same room where your regular campus mail is delivered and picked up. Mail carriers will not pick-up shred boxes from other locations.
Make sure you have the authority to dispose of records. To ask questions about disposing records, contact Records staff at recordsmanagement@umsystem.edu. Any boxes received at the Records Center with the Green Records Destruction Label attached will be disposed of.
Records which are not of a confidential or sensitive nature may be disposed of by means of regularly established departmental practices for handling recyclable or wastepaper.
Destruction pick-ups are available on Tuesdays. To schedule a shred box pick-up, send an email to mailroom@mst.edu. Due to the high volume of shred pick-ups at this time of year, the number of shred boxes we are able to retrieve at one time may be limited to 5 boxes. For this reason, please consider scheduling your pick-up before the boxes cause you inconvenience.
